How to Obtain Copies of Your Medical Record
Our medical records department complies with Federal Regulations - Health Insurance Portability and Accountability Act (HIPAA) therefore the request must be received on a HIPAA compliant authorization form before we can release the health information to you or to your new physician. Please complete all the areas on the authorization to disclose protected health information. Click here to print authorization form consent. Mail or drop by the completed authorization in order to request medical information.
There may be a charge involved if your request is for legal or insurance reasons. The current fee is $2.00 per page. If your request is for further medical treatment, there is no charge.
It can take several weeks to receive the requested medical information if you were a patient more than a few years ago since the information has to be retrieved from the archives.
